    Licenses. Check to see if your condition is under the 21 qualifying conditions.
  2.   Fill this out; always speak to your primary care doctor. Even if they may be in a sup
    portive stance, they may be able to save you in fees. If there are any medical
    records that you have take them with you or request them if you have to use another
    provider. If they are unable to sign for you, look at some local providers around you
    area who can. If this is hard to find there are usually various websites and online
    groups who are willing to help point you in the right direction.
  3.   When you are ready to send off, it is always a good idea to save a copy for your own
    records and give the DOH at least a 30 day period to get this approved. From past
    situations it may even be a good idea to send it in certified so there are no
    discrepancies or lost in the mail delays.
  4.   Once you receive your card, it is always good to touch base with a Nurse or
    Consultant for specialized treatment plans to suit your health!
